TO REGISTER FOR PRE-KINDERGARTEN:

· Child must be 3 (three) or 4 (four) years of age on or before September 30, 2012.

PARENT/GUARDIAN IS REQUIRED TO PROVIDE:

1.    Child’s Official Birth Certificate with seal and registry number.

2.    Two proofs of residency in the City of Trenton (Current to date)  Monthly billing statement from PSE&G, telephone, cable TV, City/State/Federal Tax Bill or Return, etc.)

**A DRIVER’S LICENSE CANNOT BE ACCEPTED

3.    Child’s Immunization Record

4. Individualized Education Program (IEP) if applicable

 PARA MATRICULA DE PRE-KINDERGARTEN:

·  Todo niño/a que tenga o cumpla tres o cuatro años para el 30 de Septiembre.  

PADRES/ENCARGADOS SE REQUIERE QUE PROVEAN  LOS SIGUIENTES DOCUMENTOS:

1. Certificado Oficial de Nacimiento con el sello y el número del registro.

2.  Dos pruebas que indiquen que usted vive en la ciudad de Trenton.  (Corriente hasta la fecha presente)  Puede traer como prueba el recibo de la luz y del gas, del teléfono, del cable y de la televisión, los papeles de impuestos estatales y federales, etc.

**NO SE ACEPTA LA LICENCIA DE CONDUCIR COMO PRUEBA**

3. El registro de vacunas de su niño.

4. Plan Educacional Individualizado (Individualized Education Plan IEP), si aplicable
